We do monthly patching using Altiris, and patch groups are configured in such a way that production gets n-2 patch set, staging gets n-1 and qa gets n(current). QA and staging goes through periodic functional and perf test cycles.

we just update to Windows 2012 R2 because of the scheduled end of support of previous OS (Windows 2003 R2).
