Workflows and Configuration. Promotion Request Terminated and/or duplicates itself

I am using Windchill PDMLink Release 13.0 and Datecode with CPS 13.0.1.5

Configuration and Maintenance. After installing Windchill PDM Link, Workflows, Lifecycles and Changes do not properly run as we start Promotion Approval requests and other functionalities. Refer to our previous case: PTC Case: C17965633

Promotion Request starts but then goes into a loop where it duplicates the request over and over. We have stopped and started the WfPRopagationQueue and the WfUserWOrkQueue and is not working.

Is this still occurring? If you have a PTC tech support case, please share the results here. Is it duplicating the object or the workflows? I suspect some misconfiguration in the lifecycles and workflows is the issue. Are these OOTB workflows?
