variable pitch thread

How to create program for variable pitch thread, can it be created in turning itself or it requires advanced mill-turn??????????? If it is done using 4 axis surface milling i can't select surfaces to be machined as whole one so the tool follows a zigzag path how to machine these type of components???? Is there any option to do this help me please..........
