Auto populating in 'Description' Field of Create Change Request

Use Case: I want few fields to auto populate in description field of Change Request

Description:

I want 7-8 fields populated in the description field while creating a Change Request. I was following this Modifying New Change Request description ? - PTC Community but can't find the files mentioned. Can anyone help me with this ?
