Report results

I have a query that shows the Routing/Process History for a workflow. For some reason it does not show all the activities in the query report and I am not sure why. It is also not consistent. Most of the time the query report shows all the activities, but there are a few where they do not show up. Does anyone have any suggestions on what to check for?
