Secondary Content being discarded

I have a document to which secondary attachments are made. It appears that when an Iteration /Set State operation is performed, the secondary content is discarded. It is left in place for the old iteration but missing from the new one. Is this behavior to be expected or is there a system problem here? any solutions?
