PSB Custom Action

I am using Windchill PDMLink Release 13.0 and Datecode with CPS 13.0.2.10 We followed the instructions described in the link: https://support.ptc.com/help/windchill/r13.0.2.0/en/index.html#page/Windchill_Help_Center/customization/WCCG_BusLogicCust_ReportGeneration_ReportSelection.html, to customize the psb report menu and add the entry for a custom menu. The result appears as shown in the attached image. I'm also attaching custom-action.xml and custom-actionmodels.xml. Can you help us understand what we may have done wrong?
