Why am I getting "Unable to set the selected promotion request state"

First need to point out I'm not using PDMLINK only using Intralink 10.0
on a test server
I have experimented and gotten this to work using "Basic Life Cycle" but
now I'm trying with a modified Life Cycle and transition states and I
get "ATTENTION: Unable to set the selected promotion request state"
Does anyone know what may be causing this?
Below is not the Life Cycle that I ultimately will use but what I'm
experimenting with as simple as possible to reduce what is potentially
causing the issue.
